List of documents to provide to facilitate your admission to the clinic
- your ID
- your insurance card and insurance contract
- your medical reports and test results
- your usual medicines including the last treatments in progress
- Your luggage

Rehabilitation and aftercare admission form
Please have it completed by your doctor for any request for admission to our establishment.
Here is how to do it:

2. Save the PDF on your computer
3. Fill in the PDF
4. Return the completed form by e-mail to readaptation(at)clineasuisse.ch or by fax to 022 436 36 42
Insurance
The clinic has agreements with most of the major insurers, which can
thus guarantee your stay in private or semi-private, according to your contract.
In the event that your insurance covers only part of your
stay, the financial terms will be sent to you before your arrival.
A security deposit will be requested.
We also welcome patients who do not have Swiss insurance, each request being the subject of a quote.
Prepare your discharge
The day of discharge is decided in consultation with you and your doctor. All administrative and medical documents will be given to you by the nurse.
Hospital costs
The invoice will be sent directly to your insurance.
Personal expenses
Telephone calls, drinks in the room or at the bar or any other expense not covered by your insurance will have to be paid on site on the day of your outing.
